to describe the entity responsible for completing, carrying out, implementing, enforcing, and complying with a
particular policy component.
Vital Care Infusion Services, LLC, a Delaware corporation, is a franchisor offering franchise agreements to
entities desiring to offer infusion pharmacy and related services within their respective markets. Use of the
term “Vital Care Infusion Services, LLC” within Apollo refers to the franchisor and shall not be confused with
terms used to describe the franchisee. Unless clearly indicated and referenced as such, Vital Care Infusion
Services, LLC is not the entity charged with responsibility for completing, carrying out, implementing,
enforcing or complying with any policy component that is the responsibility of the franchisee. The descriptive
term used to identify the franchisor shall be indicated as Vital Care Infusion Services, LLC throughout
Apollo.
The terms “franchisee,” “franchised pharmacy”, “pharmacy” or “company” are used to describe the entity with
which Vital Care Infusion Services, LLC (franchisor) has entered into a Franchise Agreement. That entity will be
providing infusion pharmacy and related services covered under provisions of the Franchise Agreement. Use of the
terms referenced within this paragraph in no way is intended to imply that Vital Care Infusion Services, LLC is
responsible for completing, carrying out, implementing, enforcing, or complying with a particular policy
component.
